Before Completing Form…
Do not use this form to report an immediate threat. If you require emergency assistance, call 911.
Why use this form?
Can I fill out this form anonymously?
Yes. But if you don’t give your name, the College will be limited in the amount of follow-up it can do.
Who can use this form?
Use this form if you are a Kalamazoo College student, faculty, or staff member wishing to communicate to the college on your own behalf or on behalf of another person. All faculty and staff — except those with a legal privilege such as mental health counselors and chaplains — must report allegations of sexual harassment.
What happens after I fill out this form?
Information on this form is sent to the Title IX Coordinator. The Title IX Coordinator will then contact the individual who has been the target of sexual harassment or assault in order to discuss their support, safety, and formal reporting options.
Is the information on this form confidential?
No. Students who would like to speak with someone confidentially should contact the Counseling Center, the College Chaplain, or the YWCA. Faculty and staff should consult with off-campus licensed mental health counselors, clergy, or the CIGNA Life Assistance Program.
What if I am a student who violated the alcohol policy?
The College strongly encourages students to report incidents violating the Harassment Policy and Title IX. Therefore, students who act responsibly by reporting to the appropriate authorities information about conduct violating these policies typically will not face College disciplinary action for their own drug or alcohol possession or consumption in connection with the reported incident.
What if someone retaliates against me for reporting?
The College does not tolerate any form of retaliation for reporting a Title IX issue in good faith or for participating in an investigation.
